The tabs table already has an INDEX on TabID, it is create to enforce the Primary Key constraint and is the  CLUSTERED index.
An index on TabName, URL or IcoFile would not help the GetTabs procedure.  This procedure selects based on PortalID.
I am however interested as well into knowing how many tabs you have to generate 2,000 reads.

i recommend you update to a more recent version. In 4.4 there was substantial work done on performance, particularly around the expensive fileid= joins and stored procedures dealing with tabs and portals, as well as tweaking cache usage.  Please take a look @ http://www.dotnetnuke.com/Community/ForumsDotNetNuke/tabid/795/forumid/108/threadid/89984/scope/posts/Default.aspx , they appear to have had similar issues that were resolved with an upgrade to 4.4

Thanks for all the advice. I've just upgraded to 4.4 and the site now runs like a rocket!
