One of the problems with upgrading from DotNetNuke 4.4.x to 4.5.x is that the new installations have AJAX extensions supported and have incorporated the scriptmanager in with the installation files so that you cannot keep the scriptmanager already on your .ascx user control files in your designer modules. By editing your .ascx file and taking out the line:

<asp:ScriptManager ID="ScriptManager1" runat="server"> </asp:ScriptManager>

your installation should run smoothly (given it was running smoothly before the upgrade).

Were all of these cases upgrade scenarios?  Any clean installs with these errors?  The script mananger code is written in the default.aspx.vb file and as long as this file has not been modified there should be no problems with a script manager being inserted on the page.  Did any of you keep your old default.aspx.vb file around for any reason during your upgrade?
