I have an idea now, that would explain everything.  Based on this post:

http://www.dotnetnuke.com/Community/Forums/tabid/795/forumid/107/threadid/123590/scope/posts/Default.aspx

I was surprised to discover that the collation on my laptop where my install was successful was SQL_Latin1_General_CP1_CI_AS, which it was supposed to be case sensitive (oops).  On our production servers the collation is case sensitive.  I tried changing the collation for the DotNetNuke database to insensitive, before running the scripts, but that wasn't enough.  It looks like maybe you need another instance of SQL Server that's case insensitive running on the same server, but I have not had time to test that.  I'm not sure I could get permission to do that either, so now I'm stuck wondering what to do...
