I'm setting up a proposal to host a small, growing company. I've been researching hosting plans and have a fairly good idea of their services and costs. However I'm not sure how to charge my client in addition to the hosting plan.

I know there are a lot of variables in charging a client for your services (time per hour, maintenance, etc), but are their any guidelines the pros have been using?

I'd appreciate the help.

For me I charge an overall maintance fee that will cover the basics of cleaning up the databases, ensuring that everything is optimized etc.  General site mainance with X amount of included "Customizations" or "Administration".  I then have an hourly rate after that for changes.
