We have the same issue.

I don't know what's happened to this project. Last release was over a year ago!? Seems like version 5.0.0 has been in beta that long as well, but it's not in the DNN release tracker.

Anyone know if it's possible to add fields to the feedback module yet?

on this page (http://www.dotnetnuke.com/Development/Forge/ModuleFeedback/tabid/841/Default.aspx) it says:

  • Ability to create multiple fields on the feedback form so that the Site Administrator can setup different questions which can be answered besides the generic one currently supported.
  • Ability to configure the View Feedback comments section and display different items. (new in version 4.4.2)
    ------------------------------------------------------

    I am running feedback version 4.4.3 (on dnn 4.5.5)
    but not sure which release is supposed to handle the ability to add fields?

I'm also using the Feedback module as a Contact Us form and I have the Send From email enabled. Is there anyway to include the sender's email address on the email message sent to my inbox? (Even with Send From email is enabled). This would eliminate having to go check the sender's email address on message display for quick reply. If anyone has a solution to this, I would really much aprreciate it. Thanks.

FROM and REPLYTO addresses are properly provided in latest version, if you need a custom form module however, you might look for forma and List module (DNN 5 only) or free Helferleinform from www.helferlein.com

feeback module displaying subject value instead of subject name in email subject and moderation report. i am using 4.4.3 module.
