I deleted all the old modules I could find but the host setting still do not work.  Here are the list of files in the bin directory

Nothing like answering my own questions a month later but....

After much research I found that downloading this file

http://www.ventrian.com/Portals/0/NTForums_Attach/DotNetNuke.HttpModules.UrlRewrite.zip

unzipping it and copying the .dll file into the \bin directory of your dnn install will fix the problem.  Here it is step by step which no one has yet posted:

1.  Download http://www.ventrian.com/Portals/0/NTForums_Attach/DotNetNuke.HttpModules.UrlRewrite.zip

2.  Copy the current dotnetnuke.httpmodules.urlrewrite.dll  to a another directory for safekeeping.

3.  Unzip the above download to the \bin directory of your dnn install.

4.  Allow the new file to copy over the old one.

5.  Login to your dnn install, go to host settings, see all is good in the world, drink beers.

Mike, thanks a grunch for this solution as I just ran into this same problem. Can you give me some more details about that dll version and where it came from?
It's Labled as:
ver 3.2.2.2145
with company: Perpetual Motion Interactive Systems Inc. (This is pre Dotnet Nuke Corp)

I just did a upgrade to 04.09.05 and this fixed my problem (whew!)
