I downloaded the DNN Starter Kit.

Q. Where is the database or the SQL code to create it?

you can find the database code in the directory /providers/dataproviders/aqldataprovider/

If you create a new website using the starterkit, the files will be unpacked in a directory of your choice, and if you run the website, the database will be automatically created (provided you have sql server express 2005 and user instances are allowed). Please check the dotnetnuke installation guide for more information, that is available for download from this page: Documentation

in /websites/providers/dataprovider/sqldataprovider.

you do not need to execute manually, this  is done by the install wizard.

I have SQL 2005 Server (not express) and I don't have user instances allowed for security purposes.

I found the directory of SQL scripts.

Q. What order are the scripts executed for manual database/table creation?

not sure what you mean

DNN autoinstalls the database script, all you need to do is to make sure that the connectionstring in web.config is correct. Please refer to the installation documentation for more info

just to be clear: there is no manual db installation option available, however, you can install using any version of sql server 2000 / 2005, by pointing dnn in the correct direction
