Hi,

I have a question regarding the module to be install on DNN.

I need to create a question form that contain the detail of the user and will be store on spesific sql database.

For example:

let say i have a clinic and i want record a patient detail

and i want to used DNN as a platform to log the patient data from the checkup.

at the end i have a record for all my patient and i can retrieve back the patient particulars.

 

Is there any module available that meet my requirement?

User Definable Tables would likely handle some of this, but you probably want something more in depth.  Products like XMod can help you develop a module like this.  At least in the US there are a lot of issues regarding patient data, you might want a professional module developed just for this.
