I have essentially the same requirement - I want to capture the answers given to a survey as well as the contact info for  the registered user who filled it out so I can follow up with them later. Does anyone have a suggestion on what (hopefully free) other module to use instead  of the core Survey module? Hopefully there's a "form" module available that collects answers in a form of user editable fields, does some validation, then captures the submitted data in a database or email message.

I thought some of the eCommerce modules might cut it - but I think it gets too kludgy.

I'm in need of the same functionality. We are setting up DNN for our board of directors and would like to take votes online but we need to track how each board member voted.
