No. It is default basic DNN installation with one default home page, no third-party elements installed.

Yes, of course. Browser is IE7. It seems that some install steps was not executed...

There are either FCK files missing (some users reported issues with built in zip) or config section in your web.config. I don't have another idea, sorry.

Did you install same FCKEditor version as bundles (2.6.3, included in provider 2.0.3)?

No FCK files is missing, I checked web.config with settings from working site. And yes, I installed the latest provider version 2.0.3 you mentioned. Thank you anyway, Sebastian.

I am sure this messed installation (I did it many times, it is reproducible) is result of case sensitive/insensitive mismatch mentioned earlier. This problem exists in DotNetNuke from the beginning (many years), is always present and is really frustrating.
