Please move me if I got the wrong forum, but I'm trying to locate a decent Contact form module, ideally one that can be site licensed. I've used Feedback Designer and was pleased with it's ease of setup, but I've been trying to contact the dev regarding upgrading / licensing questions and haven't gotten a message back. So, my question to the community is:

What module do you use for 'simple' contact forms?  Ideally I'm looking for:

Custom layout (prefer token-based templates)
Ability to setup multiple send-to addresses (drop down ex: Sales, Marketing, Management, etc)
Ability to specify a FROM address (easier to whitelist one address and send all feedback from there)
CAPTCHA
Site / Developer License

Hi Wells

I'm not sure about what licensing you are expecting or wanting but I've been using OpenWebStudio to make various forms that will easily do all that you want (though I've not touched the captcha - not been a prob with no "submit" button).

There's an example configuration here too which should help.

Upcoming DNN 5 will come with form and Lists module packaged, which allows to implement custom forms - though multiple adressees are not supported atm.

For DNN4 there is free helferlein form at www.helferlein.com.
