No, it is not currently possible to add custom fields without reworking the Feedback module code. When I updated the Feedback module last spring I decided not to support this capability as it greatly complicated the amount of work that an admin user would have to do to configure what is intentionally a rather simple module targeted to a specific use. The "core" Form and List module does allow the definition of custom fields and has the capability of sending an email when a form is submitted. You might also want to look at some of the other form modules available in both the DotNetNuke Extensions Forge and on Snowcovered.com.

If you have not already logged your need as an enhancement request on the Feedback module's CodePlex Issue Tracker (http://dnnfeedback.codeplex.com)  I would suggest that you do so that it might (possibily) be considered in a later version of the module.

I gave Form and List a shot and it was perfect. I've got the full Onyaktech suite it just seems like the H2O module requires too many hoops to jump through for a simple Contact Form.
