IMHO, this goes behind the scope of a user defined table. Have a look at xmod, that might be able to create such a mudle, otherwise you will need to create it yourself. sorry.

Another commercial 3rd party module for creating forms which you may like to investigate is Enterprise Forms by Ethuongmai. You can check it out at http://www.longnhi.com.
