How can I add the CREATEDDATE token to the form.html in a way that allows the repository user to edit the Created Date field of an article?

I tried adding this token to the form.html and form.xml files but when I apply the theme, instead of a way to edit the created date I get nothing

Can anyone help?

CreatedDate has been implemented as a 'system maintained' field. The server date/time value is inserted into that field when a new repository item is uploaded. It is not editable. None of the underlying code or stored procedures are designed to allow you to update that field manually. If that is truly a requirement for you, you would need to edit the source code and stored procedures and rebuild the module.  Please note that the module is a ..net 1.1 assembly so you would need VS 2003 in order to modify and recompile the module.
