Let me preface this with this is the first time I have used this module. 

I was able to install the module correctly and get a form up and running.  However, I would like to attach a template for it in order to provide some static text and instructions in my own format.  I found the XSL files in the module but, I cannot find where I can set the template.  Also, I cannot find the "Display Settings" referenced on the help page (see below).  Perhaps I am just missing something completely obvious.  I am using DNN v5.01.04 and F&L v5.0.3.

Two very powerful options are available for helping you work with XSL transformations. These features are especially useful for debugging and learning more about XSL transformations syntax.

  1. Choose Display Settings > Rendering Method: XSLT using built-in stylesheets, then select a XSL File from the dropdown list
  2. Click Show XSL source code to view the XSL

-or-

  1. Choose Display Settings > Rendering Method: XSLT using self made stylesheets
  2. Click Generate New
