Changes to Copy Page were introduced in 4.6 (I belive) that allowed finite control how modules were copied.  This allowed you to specify New - Copy - Reference.  Am I missing how to have that control when Adding an Existing Module?  It appears to default to 'Reference' and I'd like to make a 'Copy'.  If it's not there it seems like it should be.

You're not missing anything, it's behaving as it should.  Adding an existing module is referencing the original module.  Adding a new module gives you a new instance of the module.  Currently there is no way to add a module and copy the settings only from an existing one.

Except when copying a complete page, or adding a page and using 'Copy Modules from'.

Along the same lines; is there a way to see whether a module has been copied, added, or referenced on each page?

Starting with DNN 5.1 you can go to the Module Settings for each module and see where else that module is being displayed (with the referenced content). Other than that there is no way to tell the original source of a module afaik
