I think your solution is working, but I need to do more testing.  What I am seeing though is the following error on every page now:

Uncaught Sys.InvalidOperationException: Sys.InvalidOperationException: The PageRequestManager cannot be initialized more than once.

Which seems to indicate that DNN is trying to load another scriptmanager after the one in default.aspx and it is failing (gracefully except for the error).

The steps (I have given) are working fine for me.
I may have to look at your code and process to advice something. DNN should not add, as you are telling DNN by adding it to the default page. At least not happening for me.

Do you have following line in your module code?
DotNetNuke.Framework.AJAX.RegisterScriptManager();
then comment it, you should not add it.

Also what version of AJAX.NET you are using? Should be 3x.

After upgrading to v5.5, I'm experiencing the same error that others here have reported:

AjaxControlToolkit requires ASP.NET Ajax 4.0 scripts. Ensure the correct version of the scripts are referenced. If you are using an ASP.NET ScriptManager, switch to the AjaxScriptManager in System.Web.Ajax.dll, or use the ToolkitScriptManager in AjaxControlToolkit.dll.

I tried Latiful's suggestion of adding the ToolkitScriptManager to default.asp but that gives me an error that I can only have one ScriptManager registered per page.  The route I'm actually looking at based on several references to similar problems found in versions prior to 5.5 is modifying the source.  I know this isn't always the best thing to do but I just need to get this working again quickly so the site is functional again.  I was considering modifying AJAX.vb and replacing the references to System.Web.UI.ScriptManager with references to AjaxControlToolkit.ToolkitScriptManager.  In theory that should have the ToolkitScriptManager added to the pages but in initial testing that doesn't resolve the issue either although I'm not entirely sure why.

The application is running on ASP.NET 3.5 and DNN 5.5.  Any suggestions on how to resolve this quickly will be greatly appreciated.  I can figure out a more elegant solution once this is up and running.

As a quick update... Apparently the first time through I missed a reference when (that's what I get for working late nights on the weekend, I guess :)). After correctly changing all the references to System.Web.UI.ScriptManager to AjaxControlToolkit.ToolkitScriptManager and rebuilding the project, it seems to work fine. If anyone is interested in the details of the change, let me know and I can provide methods names or line, etc. Hope this helps someone else.

Latiful, did you get a resolution on this. I'm trying to use the latest AJAX toolkit 40412 because it has a bug fix I need and I can't get it to work on DNN 5.2.3.
