I am new to dotnetnuke,i have installed dotnuke on localhost, and everything works fine also downloaded training material from the site on Module Development and Portal Customization.

One question i have is(which might be because of actually understanding what dotnetnuke is for) can i use it as a framework to build a application like say a Hospital management system, Do i have to custom develop Help Desk , Electronic Medical Record,Pharmacy  etc.. as seperate dnn modules ?

Precisely Can DNN be used for Web Applications or only for internet facing portals?

Yes you can.

DNN is an ideal framework to start building applications, to begin with you have to get your head around the way DNN works and what it can do for you, but after that learning curve it can be a great time saver.

My company uses an in-house web application based on DNN to do all own automated invoicing, project tracking, task, estimates, accounts, etc...

Fort an open source application have a look at NB_Store    -  http://nbstore.codeplex.com.  

It's probably not extact what your after, but it'll give you an idea of what can be done.
