I have made a custom module that creates 1 row per module instance... Can I delete this row on the module instance when I delete it?

Your post subject mentions "module settings" so I assume you are asking how to delete an entry made to the ModuleSettings table and not an entry made to a custom table created by your module.

Entries that were made to the ModuleSettings table will be automatically deleted by the DotNetNuke framework when the deleted module instance is manually removed from the recycle bin, not when a module instance is deleted from a page.
