Hello,

Is there a way to add custom fields to Feedback core module? I'd like to use that format but also add telephone, address, etc.

Is that possible or do I have to get a third party module?

That is not possible with the current Feedback module, version 4.4.3. You may want to look into using the core Form and List module which does allow you to create custom forms/fields.

I have just been appointed as the temporary lead for the Feedback module project and will be soon uploading to the project's CodePlex page a beta version of Feedback 5.0.0 which add fields for all of the sender's contact information (including telephone) and provides for templated display of published feedback as well as improved moderation/management of feedback. The added fields can be marked as required or optional or hidden. If the sender is authenticated, those fields will be populated from his/her DotNetNuke user profile.

At this time, it is difficult to estimate when the new version of Feedback will be officially released for production use as it will have to pass through the release tracker testing process.
