I found the issue, there may be an issue with the package installer in a webfarm environment.

On a lark, I looked in the bin directory and found that the dll for the module had not been updated for over four hours!!!  Regardless of the fact that I had installed multiple package versions to try an address the situation.  I hand copied the dll into place on the share and restarted the web servers, things seem better now.

Thanks for looking all the same.
