Hello guys, could you please recommend me which form module (including commercial once) will satify below requirements.
I checked several (Custom Form Creator 1.9 (with ML support); Drag and Drop Form Builder ; Custom Forms Pro; XMod Pro 2.7 - Forms and Views for Databases; Dynamic Forms 3.4),
but I could identify covering ALL requests below.

- storing records in DB (must have)
- view stored records in form (must have) 
- Edit/Add/Delete of row (must have) 
- Get DropDownList list of items from SQL running query. e.g. select list_ID, list_Text from table_list_of_categories (must have)
- Get DropDownList list of items from SQL running query with dynamic paramaters from the form. e.g. select list_ID, list_Text from table_list_of_categories where user = @userID (should have) 
- allow user to edit rows which have been added by him/her-self only (must have)
- set up email notification (could have) 
- set up some items as not editable - only viewable. e.g. save_datetime, update_datetime, Create_user, update_user (could have)

I know it's not in your list and I can only speak to what I have experience with.
The R2i OWS module would meet all your requirements but you will have a small learning curve with it.

From my short experience R2i seems to be great for smaller things. I don't want to overcomplicate it. Otherwise I'd code it in ASP.NET and it'll give more control and flexibility.

I actually found another form generator which seems pretty nice, but I haven't check it in details.

You can also have a look at DotNetMushroom RAD. It is very intuitive and powerful and you can use it to build both forms and whole applications. In fact we use it for all our DotNetNuke module needs. You can find more information HERE. There is also a trial version on Snowcovered

Enterprise Forms is the big boy on the block with its robust customizable workflow and features for conditional visibility (pages, elements, workflow steps) and conditional binding (elements) basing decisions on such things as DNN role, Enterprise Forms internal role, a form element value, etc.

Unfortunately, the supporting vendor/developer seems a bit on the shaky side at moment. But this module has been my mainstay for some very sophisticated web capabilities.
