Hi, It would be useful to include a reference to the feedback form in the email notification. Maybe an option to enter a name for the form eg Website (Name) Enquiry Form, so that the receiver knows a bit more what the email is about.

I would start by simply adding a link to the feedback page. I'm not sure if the module accepts querystring parameters though to populate fields via the Link

I fear, Chris did not properly read the question ;) Unfortunately, the email is not template based, i.e. it is dynamically composed in code behind. You may modify SendEmail in Feedback.ascxx.vb, but you will need to redo it on any upgrade. I suggest posting an enhancement request into the issue tracker at dnnfeedback.codeplex.com
