How/where do you insert an icon that will appear in the browser/bookmark/favorites next to your website address?

it's known as a favicon, see http://www.favicon.co.uk/ for more details and a generation tool. Also, please see the wiki http://www.dotnetnuke.com/Resources/Wiki/tabid/1409/Page/Favicon-icon-per-sub-portal/Default.aspx for details on how to make favicon's portal specific if you need that also.

where do I insert the html code for the favicon? is it in the skin?

you don't need to, you just drop the favicon.ico in the root folder and it will appear for all sites - if you want to have portal specific versions you put it in the portals folder(s) as per the wiki article. DotNetNuke will check for the file's existance and automatically build the relevant link.
