I have changed the host and admin passwords and do not allow user registration. Any ideas how the defacing took place?

a quick investigation of this group shows that they've attacked a number of different server OS's, and no one particular web application. This profile typically suggests a server level hack e.g. one that has missing Microsoft security patches or missing patches for a server level tool such as plesk. I recommend you run windows update to validate that you are patched, and run a virus scanner on both the server and your local desktop to ensure that you have not contracted a virus (virus's can install keyloggers that detect FTP details and allow hackers to log in via FTP as though they are you - as such I'd also recommend changing any FTP and sql server password details)
