Today I performed 4 site upgrades to 6.0.  3 went flawlessly and 1 failed.  All on the same server.  All running the same ASP.  On the 4th and failing server I got the '/' server error, can't recall the exact error at the moment.  So prior to the upgrade I performed a backup of the site (I'm not that dumb) and reverted back to the backup.  The site opens fine, but when I log in I get the following error:

It only appears to affect the top menu as I'm able to edit contents, move around, but the error persists.

I'm not sure why this one site failed.  I've attempted the upgrade 3 times with the same results each time on this site.  Like I said earlier, the 3 previous sites upgraded properly.  Any idea how to fix my current issue on 5.6.2 and what may have caused this site to fail the upgrade to 6.0?

Parameter count mismatch is usally caused by either the database upgrade hasn't run properly (see .log flies inside /providers/dataproviders/sqldataprovider folder) or the .dll files not properly being replaced in /bin folder, please check by date and version info.

I'm stumped completely on this.  After restoring back to 5.6.2 I get the previously mentioned error.  At first I thought it was no big deal as it appeared the site was still working.  But now I find that the file manager is having issues as I can not see any files in the image directories, even though they are there.

I took the step to try a fresh 6.0 install, but I can't get past the config database page.  No matter if I try integrated or providing the user id and password to the database.  It just won't go past that screen.

I'll continue beating my head against the wall trying to figure out why 3 site upgrades worked fine and this one failed.

One more thing.  What would I be looking for in the sqldataprovider directory?  Also, is there a way of testing SQL from within Host/SQL?
