The real fix is in the core, which had issues from VB to C# conversion in 6.0, which has been fixed in 6.1.1. Here is the changeset in codeplex: http://dotnetnuke.codeplex.com/SourceControl/diff/file/view/63607?fileId=986777&olderChangeSetId=63245

The other possible fix mentioned earlier in this thread is in the AD Provider, which is a separate project and hosted here: http://dnnauthad.codeplex.com. That fix will get-by the current problem and you can do it yourself.

is there any way to get an updated copy of the dll before 6.1.1 comes out or get an updated version of the ad provider? I don't currently have a dev machine set up and would like to get this fixed before the end of November.

Just to clarify on the impact, AFAIK problem happens with new users only in 6.x+. Any user that was created prior to 6.x and have logged-in at least once priot to 6.x will not have this problem. However, if user's password is changed in AD and then logs in DNN, will run into this issue.

Also, the problem doesn't happen on automatic logins.

Thanks Ash for looking into this while I've been swamped @ work. FWIW I cannot replicate this problem. Just created a fresh DNN 6.0 install and logged in and out a dozen times with a test AD user and every time it logged me in.

That's surprising as both QA person and I were able to repro it pretty quickly, along with other peopple in the community. Try with a brand new Ad user that has a password. In the very first login, it creates the user account with AD password. In the second (or any subsequent), it compares the typed-in password with what's stored, if they match, it creates a random password and saves in the database. The problem occurs in the second login, where it changes the password. I don't know the logic for that though.
