Good day,

I added the form and list module to my website I am busy creating. When users want to add/edit files, the "Upload image" option is too complicated for them. Is there a way I can set or change the code, so that only the "Browse" and "Upload" button shows?

Will appreciate any help

unfortunately, the module is using DNN core "UrlControl", which does not support customization. I prefer using a dedicated upload like the one from www.oliverhine.com and displaying all images or documents inside the upload folder - depending on your concrete needs.

Where can I find an online request form with an upload option in the form?

I tried this module, SWfUpload for DotNetNuke 5.1.x, but it doesn't have the functionality I need

swfupload may be used to upload (even large files), but it does not include other fields. What are you precise needs?

Yes I noticed that when I tried the SWfUpload for DotNetNuke 5.1.x module on my online request page.

I am looking for a form that has the following options:

- drop down list boxes
- multi-line text boxes/rich html text box
- upload functionality
- radio buttons
- and single label option
- with submit button

What I want is for users to be able to attach their documents/files when completing the online form, so when they click submit, the attachment gets sent with their submission.

I tried 7 different form modules but non of them has the options and functionalities I am looking for.
Some modules I can't edit up to a point, for instance I tried the Easy Form module, It worked well but I couldn't organize the fields in the order I want, cause it doesn't have that option...
