When you copied the unzipped upgrade files to your server, this should have place a copy of Telerik.Web.UI.dll in the bin folder on your site. Can you check for that file? If it is missing, something has gone wrong either with the unzip or the ftp to the website.

I think we may finally be getting closer to your solution.

The upgrade zip file from dnn only has reference to this file:
DotNetNuke.HtmlEditor.TelerikEditorProvider.dll

There are no other Telerik files and no Telerik.web.ui.dll

I have donwloaded another copy from DNN (this morning 27 sep 2012) and there is definately no other Telerik file in the version 5.4.4  upgrade zip.
I have checked other upgrade versions for this .dll file (here are the results):

5.4.2 - same as above - only DotNetNuke.HtmlEditor.TelerikEditorProvider.dll in the bin folder
5.4.1 - same as above
5.4.0 - same as above
4.9.5 - same as above
6.1.1 - same as above + DotNetNukeRadEditorProvider.dll

I have checked my development server installation and it is there.
- Can I simply copy and paste my Telerik.web.ui.dll file into my live server bin folder (along with the Telerik.web.ui.skins.dll and the RadEditorProvider.dll) and upgrade as normal?
Or are there any other files I need? (maybe I could copy everything I dont have from the bin folder on the dev server)

As for the other problem - permissions with the repository it has full asp.net and network service permission access on the folder and root folder - so I don't know what else to do with this problem.
- I know I've customised it a lot, perhaps I could delete the folder or the templates during the installation?
and paste them back afterwards?
or upgrade the repository module first?

Is the Telerik.Web.UI.dll you have the same version as referenced in your web.config file? If yes, copy it onto the site bin folder - if no, change the web.config to reference the version you have, and then copy your version to the site bin folder.

Get the version information by right clicking your file and choosing properties, then the details tab.

The problem with copying a higher DNN version's dll's from dev to the site is that when the upgrading web.config file references a lower version of the dll it will still fail to find the correct dll and cause errors during your upgrade.

Personal preference, I would work through the errors.

Your point 2 - its not a web files permission error.

The errors are coming from a failure to find the named stored procedures in the database or insufficient database permissions to effect the changes. This could be caused by database owner name being different from the expected dbo. , it could be that you use an object qualifier in your database and this has not been taken into account in the scripts.

It may help to either upgrade the repository first OR just do a repair install of your current version to see if that recreates the < possibly missing > stored procedures.
