I do my own DNN stuff so I can't speak to the first question but I will on the second.

Upgrades are touchy with most software this customizable since you never know what the developer did to create custom modules. This isn't unique to DNN though; I have 2 apps internally that allow for a lot of custom code and upgrades can cause issues with them as well.

For the most part I don't have a lot of issues during upgrades but I'm very careful to test them prior to upgrading my live site. This should be the norm anyway, even if you don't have any custom programming in an application.
