First for upgrading to newer Versions e.g. 7.1.2 there are a suggested upgrade path on the wiki.

To Upgrade DNN 4.x to a new Server I would use Windows and SQL Server 2008 or higher.  Backup files and the database and restore them to the new Server.  After restore the database map the database user with the database login user using sp_change_User_Login on an SQL Query on SQL Management Studio.  Basicly the Server must Support .NET 3.5 for DNN 4.x and should be changed later (DNN 6) to .NET 4.x.  Also on DNN 6.x you should migrate the database to Support SQL Server 2008 (100) compatibility mode.

To configure the new server check the install instructions and requements for the Version you using or you want to upgrade.

Migration to a new server (IIS and SQL) should be not much different than any other website, if the name of SQL Server changes, you'll need to adjust both connection strings in your web.config file. Also make sure, the web server is running same version of .Net framework.
For upgrading DNN you will need to do it in multiple steps via DNN 4.9.5, 5.2.3, 5.5.1, 5.6.8, 6.1.5, 6.2.9, 7.0.5 to 7.1.2. You may follow steps in my Upgrade receipt at http://www.deutschnetnuke.de/tabid/186/Default.aspx. Before upgrading to DNN 7, you need to switch to .Net 4 and make sure, compatibility level of the database is set to 100 or above.
