Yes, there are two "Tags" in the list, but I see no way to delete them from the menu. Do I need to do it though the database?

Click on one. Does it go to an edit screen that offers Delete?

The list menu shows "Create New Vocabulary" as the only option.

If I click on the edit pencil for any of the entries, I have "Update", "Add Term", and "Cancel" as available options.

that's expected (there is no delete option) - you will have to manually delete the entry from Taxonomy_Vocabularies (check Taxonomy_Terms to ensure you delete the right one). As there is no way for this to occur with the UI and we have no other reports of the same issue, I would suspect some 3rd party extension incorrectly added the additional vocabulary
