Ideally I would like to have the subject field hidden and pass along "Contact Us Form Submission from Name". I will settle for "Contact Us Form Submission". Not sure how to do that. I went to "Edit Category & Subject List" Gave a subject name of "subject" I gave a value or "Contact Us Form Submission" It passes the name rather than the value. So I'm stumped... Thanks

It is the Name that is sent in the subject line - so just set that as needed. Then set the Subject value accordingly in the module settings, and then set the subject field to hidden. It isn't possible to include the 'from Name' at this time. Though you can add this as a request on the CodePlex site.
