Richard, a couple of changes have been made regarding PortalSettings being loaded - this could be the cause of this issue as well.

More details:

We tried to disable web farm synchronization and output caching, and started over with the 3 web servers serving correctly for about 2 hours before one of them started again serving empty pages.

 The portal has en-US and fr-FR locales installed with only the later enabled. Do you think it could be related?

Could the new Versioning features have an impact too? For instance, in TabVersionBuilder, l.311, if for some reason, cache corruption leads to versioning getting enabled, I see that an empty list of ModuleInfo objects could be returned.

@Jesse - It sounds as if you have developer skills. Might it be feasible for you to isolate - ie take out of the web farm a misbehaving server?

If so you might consider using Visual Studio to connect to that isolated server and use remote debugging. Not a great experience on release code, but it could let you see where the code actually goes and that might lead you to why it's serving these empty pages.

I don't think it's a web farm issue, as the issue persists on my dev box as well. It's a frustrating thing to isolate too since it takes 20 minutes to test each time :) If it's a portal settings issue, then why does the page load with the correct skin etc? The only thing missing are the modules added to the page.

@Andrew - are you a developer?

I didn't realize this can be reproduced on a dev box. It's open source. You could grab the source and see if you can spot where it takes a wrong turning and avoids rendering the modules.
