I'm having the same problem.  There is also a place in the "web.config" file with the line:

AvailableToolBarSets="DNNDefault,Default,NoGallery,Basic"

I've added my new toolbar name to this line in addition to editing the "fckconfig.js" file but still no luck.

1) Add your new toolbar to fckconfig.js

2) Include the toolbar name in the AvailableToolBarSets property at web.config

3) Clear your browser cache and your DNN cache (This reloads the fckconfig.js)

4) Use the custom settings dialog for FCKeditor and the new toolbar will be listed at the toolbars grid. If not, clear your custom setings and reload that page again.

Thanks.  Apparently I had done everything right, but had not cleared my cache, so it wasn't showing up.  Thanks for the detailed post.
