I completely reworked the Survey code logic for the DNN4 version and I think it might solve some of the problems with the DNN3 version that I have been unable to reproduce (that's why I can't fix them).

Unfortunately, I wont have time to back-port the code to DNN3 anytime soon. Does anyone else want to take this on?

You can download the DNN4 version of the Survey module using this link.

Before you upload it to an existing DNN4 site you have to remove these two assemblies from the "\bin" directory of your existing DNN site.
    DotNetNuke.Modules.Survey.dll
    DotNetNuke.Modules.Survey.SqlDataProvider.dll
