I know that I am a late poster to this thread, but I have found that the proposed solution of changing the SQL Collation does work. If you change the collation of the database to  Latin1_General_CI_AS when using a SQL 2005 server will fix the issue. Another item of note is that in some of the documentation it is not crystal clear that Network_Service MUST have 'Modify' right or you will get the error accessing .log file errors.

As a newbie, I may have missed some other steps as I am working through the timeout issue now, but at least I got past the DB_Create issues.

In DNN 5, the Sql Server db must still be case insensitive.

I was having the same issue and found that the objectQualifier was not set correctly in the web.config file
