Hi,

I tried doing a clean installation of 4.5.0 and the "Windows Authentication" module is present in the Modules table.  So something must've happened in my live database that led to the deletion of that row.  It seems benign though, since the site works (as far as I can tell anyway).  I'm going to try and just add that row back in manually and cross my fingers.
