I have tried to install a new 5.01 website, how can I import the content/pages from the 4.09.01 site? I installed the DNN4_LocalizationExtensions.1.04 on the new 5.01 website, seems it works on the defaul portal. I created a another portal by using the template which is exported from the 4.09.01 site, but seems the DNN4_LocalizationExtensions.1.04 has error when working on the new portal.

to upgrade a 4.8.0+ site to 5.x, start with a backup of site and database.

unzip the upgrade (or source package if used before) to overwrite existing files.

Browse to your site and watch it getting upgraded. done.
