I agree, that a misconfiguration or suboptimal developed module can cause memory leaks, leading into out of memory as well. Many issues can be caused by various reasons and I usually try the most obvious and easiest first. Checking the error logs will give additional hints, but there is no guarantee to find the issue without getting hands on the system itself or being able to create a simulation system from a backup for local analysis.
