I am trying to setup Vanity URLs (aka www.mysite.com/youth) via Friendly URLs and can't quite get it to work.  Here is what I am using in my SiteUrls.config:

    <RewriterRule>
      <LookFor>.*mysite.com/youth(.*)\Default.aspx</LookFor>
      <SendTo>~/Default.aspx?TabId=69</SendTo>
    </RewriterRule>

Any idea why this isn't working?

This is a long shot, but have you put a / rather than a \ before Default.aspx?

Thanks for the reply.  I just tried it with the forward slash before Default.aspx.  Now I have this:

<RewriterRule>
      <LookFor>.*mysite.com/youth(.*)/Default.aspx</LookFor>
      <SendTo>~/Default.aspx?TabId=69</SendTo>
  </RewriterRule>

However, still no dice.  for clarification www.mysite.com/youth doesn't work and www.myseite.com/youth/default.aspx does.  Ideas?

Make sure there are no rules listed before your rule that might pickup that url that you are trying to match.

you cannot virtualize folders without modifications on IIS setup, you might be able to use www.mysite.com/youth.aspx as an alias and redirect. Also try the human friendly urls rewriter of DNN 4.7.0 (Stefan blogged about)
