Don't know if this issue is still under investigation. But we are running DNN 4.8.3 and we have encounterd the same issue. The last three month's we lost the link's between 30+ documents en the item's in the file-table. FileID = xxxx records links disappeard; found back the files in the file-table under a different (higher) fileid. Also the Site-logo disappeard, although it's still exists in the DNN-directory.

It may be a matter of directory-rights. Because in some cases when i tried to repair the link I got a message "You don't have permissions to see files on the site" although I was logged under the HOST account.

Hello,

I am running DNN 4.9 and still have this problem.  All the images added using the media module are disappearing every now and then.  This is very frustrating. 

Below is the error message from the event viewer:

Any updates or work arounds for this problem pls post.

I'm having the same problem in 4.9.1. I'm having a hard time convincing my clients to stay with me when I can't tell them why their critical files are disappearing off their sites and they have to keep uploading them again. Any help or insights anyone could offer would be greatly appreicated. Thanks.

Do you have file sync enabled in the scheduler? If so try disabling it, if not try enabling it.
Are you doing a manual recursive sync?
Also consider upgrading to 4.9.2
