I'm sorry but since I upgraded to DNN5 I have all sorts of issue and this is just another one. I have been pulling my hair out for the last hour because of this stupid bug. I can't believe it's not fixed yet.

Here is the scenario: Set module to display on all pages, than change it not to display on all pages and finally change it back to display on all pages. If this is the case and you having problems all you need to do is to empty the recyclin bin before you set the module to display on all pages second time. It is similar with pages, you can not create a page with the same title as the one in the recycle bin. I never though about this as a bug -  just assumed this is how it works :)
