Please try to stop the application pool, then call the domain on a bowser - you want get a error.  Restart the application pool, then retry to open the domain in an browser.

Is it a DNN 5.4.0, 5.4.2, 5.4.3 or 5.4.4 install ?  Have you used a objectqualifier and/ or a other databaseOwner?  Have the database user dbowener right on the MS SQL server?  Have you used SQL server 2005 and above ?

Had you any issues logged on the install?

What are the .Net configuration and your physical folder and file permissions incl. all Subfolders? Using Network Service as the application pool user? 
Is it on Win XP, Win Vista, Win7, Windows Server 2003 or 2008 ?

are you using a proxy server, intermitting Antivirus software, redirect solution?

 Version is 5.4.4
SQL account is DBOwner, SQL2008
No issues on install
Running on Windows Server 2003 x64
App Pool is Network Service
I am not sure what you want for the rest of the questions. .Net config what? Do you need the list of all accounts with access to DotNetNuke Directory, or are you looking for a specific account and permissions? Not sure what "objectqualifier" is.
