What are the proper steps for setting up DNN to use Windows authentication?

DotNetNuke is just an asp.net site so you set it up the same way i.e. set authentication to windows you're done (http://msdn.microsoft.com/en-us/library/ff647405.aspx) . This simply means that the account of the service runs the site - but if you want is the site to run in user context then you'll have to enabled impersonation. Finally if you want to integrate with windows roles/permissions, then you have to install and configure the active directory provider.

I configured the AD provider and it appears to work.   I can use my Windows login to login to the system and have appropriate permissions.

However, I don't want the user to have to login.   I set my authentication settings for the site to the following but I just get:  You do not have permission to view this directory or page.

Anonymouse Authentication: Disabled
ASP.NET Impersonation: Enabled
Basic Authentication: Disabled
Digest Authentication: Disabled
Forms Authentication: Disabled
Windows Authentication: Enabled

Is this not the correct combination to get the user to automatically logged in based on their current username?

it appears correct, but it looks like you havent granted the users permissions i.e. when you run with windows authentication you need to grant the users read access as requests are no longer funneled through the anonymous webuser. There is a user guide at http://www.dotnetnuke.com/Community/Extensions-Forge/Provider-Authentication.aspx that covers common installation scenarios.

Where should these permissions be granted?

In Windows File Explorer, "Everyone" has full access to the DNN installation folder.
In DNN, my user is a super user so should have access to every tab.
