I have a Windows 2012 server with a few instances of DNN. Every instance has had some issues recently where clicking on save/update in a module (even the HTML module) will act like it saves but when the page reloads the content hasn't updated. If I click "Clear Cache" or recycle the App Pool in IIS then I will see the changed have in fact been made but for an end user this is very frustrating. I can give many recent examples of this behavior such as renaming a page and the menu not showing the change until I clear the cache or restart the application. 

This is happening on DNN 7.1.2, DNN 5.6.8, and DNN 6.2.4. 
I cannot think of anything that changed lately on the server that would affect this and don't really know where to start troubleshooting the issues. These instance have been running for years and this only happened in the past month or so. 

If anyone has some ideas about this, please let me know. I can provide more info if needed to troubleshoot.

Please check Cache Settings of each module and disable Caching or check Schedule to enable delete the Cache.

I don't believe I have ever changed this value in the scheduler:  DotNetNuke.Services.Cache.PurgeCache which is currently set to 2 hours. The scheduler is set to: Timer. I am even trying it with performance set to No Cache at the host level.

It almost seems like clicking on a "PostBack" link just doesn't work sometimes. I click on "edit content" just to open the editor and sometimes I have to click on it 2 or 3 times before it actually open the editor. The page will refresh but just stay on the page view. This is happening in multiple modules, not just the HTML module. I also checked the Event Viewer which shows no errors for this behavior.

I just did this: Open edit content in an HTML module then clicked "Cancel" which should take me back to the page view but it reloads the page still showing the editor. On the third try it finally worked.  This leads me to believe it is something other than a cache issue.

Is there anything in IIS or on the server that could cause this problem?

please check server configuration for dynamic caching, it sounds like an issue here.
Do you get any related items in DNN or Server event log?

In IIS I turned off Output Caching and verified in DNN Host Settings that it is set to No Caching. Then I restarted IIS.

But, the same things I already mentioned are still happening. 

It seems random when I am just clicking 'edit text' or 'edit content' on a HTML module it doesn't always go to the edit view but will reload showing the same page view. And then as before if I make a small change to the text it isn't always updating until after 3-4 attempts.
