Max, portalID Null means host portal, these seem to be associated with superusers or users deleted from all portals.
Does Folder 47 has a portalid?

No folder 47 has NULL in portal ID and it's the right user folder. I also tryed changing the image but with no results. Also the image shows correctly (even the new one).

This is the error summary:
AssemblyVersion 7.2.2 PortalID 4 PortalName MCL Emilia-Romagna UserID 1 Us

The "Us" make me think this could be tied to some localization issue? (I don't have localization enabled for the site BUT I use Italian language).

But as I said the "index" here: System.IndexOutOfRangeException: UniqueId in System.Data.SqlClient.SqlDataReader.GetOrdinal(String name) seems more like there is no UniqueId column at all in the retrieved recordset. The error seems to be in /profilepic.ashx?userId=1&h=32&w=32 so it should be a core module right? Not something influenced by external modules or skin.

UPDATE: it seems the error is tied to you turbo scripts Sebastian. I had this doubt so I tried on a clean install.
I inserted a picture for the super user. went in event log, refreshed it a couple of times and all was ok.
I then run your TURBODNN722 script (not yet the schema only the turbo script) inside Management Studio.
Then went back to the even log, refreshed and the exception was there.
