I'm having the same problem importing a 3.x version of DNN to 4.8: 
A critical error has occurred.

Violation of UNIQUE KEY constraint 'IX_FolderPermission'. Cannot insert duplicate key in object 'dbo.FolderPermission'. The statement has been terminated. 

What do you mean by root folder permissions - I have set the ASP.NET permission to FULL for the root of the website - is this what you mean because it's not working for me.... or is it something to do with MSSQL......?

The problem is that some stuff in the Database changed for 4.5 I think.

I solved the same error by editing the template before (don't know the details anymore)

I am not able to find  http://www.dotnetnuke.com/Community/B... link because of the upgradation to www.dnnsoftware.com. Will you give me the new link of Erik blog about this issue.
