Use Transitional, Strick will fail to load properly cause it won't allow for backwards compatiability, which DNN needs.

You just place the doctype.xml in the same root folder as the skin, so right along with the skinname.css / skinname.ascx / skinname.html / skinname.doctype.xml. Would all go in the same folder location.

How do I tell if the browser is loading the .doctype.xml file properly? I didn't notice a difference even when I open the default.aspx source view in internet explorer.

Do you know how to change the top, left, content, right and bottom widths using css?

Choose View > (page) Source in your browsers menu

The source is in the first line.
