I tried to take a look at Nuke Feedback, but is it just me, or is the navigation at Orizonti.com kind of jacked up?  Another back sign was when testing their online demo of the form, I got a critical error after hitting send...

The developer is still there and has indicated (for a year now) that development will be continuing for his modules, but it has definitely been static. I'd really like to see some updates for the RSS module. The feedback module does however work and I've got it running on 4.8.1

You could try to download "Dynamic Form Module" from http://www.thinktron.com/, it's free, only need to register.

Quiet simple but well thought module, the only issue i am experimenting with it is that i have not been able yet to customize the IDs for my field. I am in it right now.

Good luck

Guillaume, any luck with this? It looks like the perfect solution, except I can't even see a button to save question configuration. Wondered if you could help me see what I'm missing here.

Please disregard that question — I moved the Save button out of a hidden area in the configure doc, and now the module works beautifully. Apart from the difficulty of styling, Thinktron's Dynamic Forms is an excellent module.

If I wanted a form that would work for newsletter registrations (this needs to allow GUIDs to be created) and I need a way from the form to send emails, these emails then need to link back to a different "form" which will authenticate them....

So which products would be able to handle this ?

You should check out Enterprise Forms.  My understanding it does have the ability to generate GUIDs as well as include form data in notification emails.
